Brief Summary
Million Hearts™ is a national initiative to prevent 1 million heart attacks and strokes over five years. Similar to the national initiative, the Million Hearts™ initiative in Colorado brings together communities, health systems, nonprofit organizations, federal agencies, and private-sector partners to fight heart disease and stroke. Stakeholders representing these groups were convened to help draft an action plan to identify strategies focusing on hypertension prevention and control. During this round table, the Colorado experience will be presented as well as a facilitated discussion for other states to share their experiences, insights, and lessons learned in using data to plan, implement, and evaluate Million Hearts™ initiatives.
