Brief Summary
Public health and primary care professionals have historically worked in isolation from each other. A push for better outcomes in population health has called for these two sectors to align and collaborate more effectively together. This roundtable will focus on activities related to public health and healthcare integration, with emphasis on public health and primary care and the role of epidemiologists. It will also provide an introduction to the de Beaumont Foundation-funded Practical Playbook, a web-based resource in development that will provide clinicians and public health officials with an interactive, nimble set of tools and strategies to support a variety of integrated models for improving population health.
