At the end of this session participants will be able to:

Explain the CHI Process requirements required by the IRS and the Public Health Accreditation Board.

Geocode vital statistics data and calculate and visualize census tract level disparities in life expectancy (LE)

Understand and assess the role of the upstream social determinants of health in driving community level population health and health disparities.

Utilize tools (e.g., logic models) and identify appropriate data sources for monitoring and evaluating interventions and assessing population health impact.

Articulate and apply lessons from the field in identifying disparities and monitoring and evaluating public health impact.
