LEARNING OBJECTIVES

At the end of this session participants will be able to:

  • Explain the CHI Process requirements required by the IRS and the Public Health Accreditation Board.

  • Geocode vital statistics data and calculate and visualize census tract level disparities in life expectancy (LE)

  • Understand and assess the role of the upstream social determinants of health in driving community level population health and health disparities.

  • Utilize tools (e.g., logic models) and identify appropriate data sources for monitoring and evaluating interventions and assessing population health impact.

  • Articulate and apply lessons from the field in identifying disparities and monitoring and evaluating public health impact.
